Physical Preparation for High-Altitude Trekking

For a high-altitude trek — whether on Kilimanjaro or elsewhere — there is probably no single right or wrong answer when it comes to fitness. But preparation makes all the difference between a memorable adventure and a difficult experience.

Equipment Matters

Your shoes, backpack and gear must fit you perfectly. After five days of walking it will be too late to realize your shoes are giving you blisters — and too late to realize that trekking may not be for you!

Test all gear thoroughly before your departure — especially your footwear.

Test Your Fitness First

Do a 2–3 day hike before your main trek. You will quickly discover your fitness level — if you enjoy walking and find a comfortable pace that doesn’t exhaust you, there is no reason why a longer trek should be any different.

Learning to find your own pace is the best advice we can give you!

Watch Your Backpack Weight

Your backpack will be with you every step of the way — avoid loading it too heavily. 10 kg is a reasonable weight, but 15 kg will feel like excess after just 3 or 4 days on the trail!

Altitude Sickness (AMS)

What is Acute Mountain Sickness (AMS)?

AMS is caused by a lack of oxygen at high elevations. It typically occurs above 7,000 feet (2,100 m) in individuals who have not had adequate time to acclimatize before engaging in physical activity. Mountain climbers and trekkers are at greatest risk.

Common Symptoms

Headache
Fatigue
Shortness of breath
Nausea
Poor appetite
Restless sleep
In severe cases, thinking and judgment may become impaired. Pulmonary oedema (fluid in the lungs) is uncommon but potentially fatal.

Prevention

1
Ascend gradually
Climb less than 500 m per day to give your body time to acclimatize.
2
Acetazolamide (Diamox)
Can be used to speed up acclimatization. Take shortly before and during ascent. Consult your physician — contraindicated for those allergic to sulfa drugs.
3
Stay hydrated
Drink plenty of water throughout the climb. Dehydration worsens AMS symptoms.

Treatment

AMS symptoms usually improve over several days without treatment. However, if they become severe:

  • Administer supplemental oxygen
  • Descend to lower altitude immediately
  • Seek medical assistance

Physical Fitness for Kilimanjaro

While a specialized training regime is not strictly necessary for climbing Kilimanjaro, the higher your fitness level, the better your body will adapt to altitude, the more enjoyable your climb will be, and the higher your chances of reaching the summit.

Individual tolerance to altitude varies greatly. Those in poor physical condition are generally more susceptible to AMS symptoms. Building a solid fitness base before your trek is the single most effective preparation you can make.

Recommended Fitness Activities

1
Weekly Aerobic Exercise
At least 30 minutes of aerobic exercise per week — running, cycling, or swimming. Build up your cardiovascular endurance progressively.
2
Monthly Long Hikes
Once a month, try to spend 5–6 hours walking or hiking to build endurance and acclimatize your muscles and joints to long days on the trail.
3
Test Hike (2–3 Days)
Complete a short multi-day hike to test your gear, pace, and physical condition before your main trek. This is the best way to identify any issues early.
